You open an RCPS case. You leave with a cause, actions, and owners.

RCPS is Root Cause Problem Solving. Site teams open a case on the failure or issue, and the evidence sits on the case: 5 Whys, Fishbone, Timeline, Is/Is-not, FMEA — plus the issues and FMECA already on that asset. Investigation Coach reads the 5 Whys and pushes back where it is still a "possibly" and asks for more detail. The case walks Draft → Assigned → In progress → Pending review → Awaiting actions. You leave with a cause, actions, and owners. Same reliability tree as FMECA, Asset reliability, and Issue Tracker, not a separate pack.
